Obverse
The Small Coat of Arms of Ukraine and the semicircular inscription НАЦІОНАЛЬНИЙ БАНК УКРАЇНИ (National Bank of Ukraine) (above); below there are the coin issue year and face value 2011 П`ЯТЬ ГРИВЕНЬ (five hryvnias) with the logo of the National Bank of Ukraine Mint to the right; in the centre there is the National Flag of Ukraine (decoration element - enamel) within a conventionalized viburnum wreath.
Reverse
On the reverse there is depicted a book of ОСНОВНИЙ ЗАКОН УКРАЇНИ (Fundamental Law of Ukraine) against the background of the Verkhovna Rada building where the Constitution was adopted; in the foreground there is depicted a laurel branch. The coin reverse bears the inscriptions: the semicircular above 15 РОКІВ КОНСТИТУЦІЇ УКРАЇНИ (15 years of the Constitution of Ukraine) and below to the left 28 ЧЕРВНЯ1996 РОКУ (28 June 1996).
